Summary of Benefits
Health Insurance
TLU provides a self-insured medical and dental insurance plan at minimal cost to all full-time employees (defined for purposes of this policy as anyone who work thirty hours or more per week). A Preferred Provider Organization (PPO) is offered for physician consultations, exams and lab work associated with physician consultations, and hospitalization. There is a $20 co-payment for doctor’s visits associated with an illness or injury provided the doctor is a member of the PPO. Dental and prescription claims are not affected by the implementation of the PPO.
Insurance is provided with a $500 medical deductible (maximum of $1,500 per family per year) and a $50 dental deductible (maximum of $100 per family per year) and a $50 orthodontia deductible. Medical co-payment for services provided by PPO providers is 80% - 20% (50% for outpatient mental health, alcoholism and substance abuse), up to $1,000 ($2,000 for family) out of pocket expense per year. Medical co-payment for services provided by non-PPO providers is 50% - 50%, up to a $2,500 ($5,000 for family) out of pocket expense per year. Dental co-payment is 80% - 20% for basic procedures and 50% - 50% for major procedures and orthodontia, with a maximum benefit per calendar year of $1,000 and lifetime maximum of $1,000 for orthodontia.
The health insurance plan also has a wellness component. Preventive care services will be reimbursed at 100% of the first $500 of covered charges each calendar year; thereafter, regular plan benefits will apply. No deductible will be applied to any of these services. Two dental examinations (diagnostic and preventive dental procedures) will be covered in any calendar year at 100% of covered charges.
The medical plan requires that authorization must be obtained in advance for any planned hospital admission or within two working days after any emergency admission. If prior authorization is not received, an additional $300 deductible will be assessed. When charges for a period of dental treatment (other than emergency treatment) are expected to exceed $500 for you or your dependents, a Dental Treatment Plan must be filed.
You and your dependents are eligible for the insurance the first day of the month coinciding with or next following employment. The current monthly cost of the insurance to you is as follows:
| | Total Premium | Employee Contribution | TLU Contribution |
| Employee Only | $574 | $110 | $464 |
| Employee + Child | $918 | $225 | $693 |
